Joint Account Termination Form

Joint Owner Termination

Effective on the date stated below, the current Joint Owners of the account identified below do hereby wish to attend the account to remove the name of one or more Joint Owners. The removal is known and approved of by all current owners. A member may not request removal of a name from the account without approval from the Joint Owner concerned. If this notice from the Joint Owners results only in a non-member Joint Owner Owners, the account must be closed and the funds disbursed unless the non-member or member is within the field of membership and join the credit union. No Joint Owner may close an account opened by the Prime Member.
